Surgical Management of Osteoporotic Vertebral Fractures

Antonio Moroni, Amy Hoang-Kim, Giovanni Micera, Letizia Gelsomini, Riccardo Orsini, and Sandro Giannini

Approximately half of all osteoporotic fractures occur in the spine, resulting in >150 000 hospitalizations every year. Non-operative treatment of vertebral compression fractures primarily consists of bed rest and medical management, which can be effective but at the cost of prolonged disability compromising the quality of life of the patient. Kyphoplasty and vertebroplasty are minimally invasive surgical procedures that stabilize percutaneous fractures using acrylic bone cement, providing the advantages of a quick procedure and efficacy in relieving pain. For the severely osteoporotic patient, the use of augmented pedicle screw fixation has good outcomes; however, more randomized controlled studies and large-scale trials are needed.
