Venous Thrombotic Events Following  Simultaneous Bilateral Total Knee Arthroplasty

This case study outlines the presentation and course of a patient undergoing simultaneous bilateral total knee arthroplasty who suffered venous thrombotic events postoperatively. Despite suffering a deep venous thrombosis and a pulmonary embolism, the patient went on to make a good recovery and had improved function 1 year following the surgery. In the article, the etiology of these events is explored, the place of the new oral anticoagulation medications is discussed, and whether simultaneous bilateral surgery confers an increased risk is debated.Adv Orthop 2009;1(4):151–155.

  In Achilles tendinopathy, there is pain and swelling in and around the Achilles tendon (AT), mainly arising from overuse. The essence of Achilles tendinopathy is a failed healing response, with haphazard proliferation of tenocytes, some evidence of degeneration in tendon cells and disruption of collagen fibers, and a subsequent increase in non-collagenous matrix. Despite the social and economic relevance of the problem, the causes and mechanisms of Achilles tendinopathy remain unclear. The diagnosis of Achilles tendinopathy is mainly based on careful history-taking and detailed clinical examination. The latter remains the best diagnostic tool. At present, only limited scientifically proven management modalities exist. New minimally invasive techniques involving stripping neovessels from the Kager’s triangle of the AT have been described, and seem to allow faster recovery and accelerated return to sport compared with open surgery.

  Obstetrical brachial plexus palsy (OBPP) is a rare disease with a reported incidence of 0.1%. The well-established risk factors for OBPP include high birth weight (>4.5 kg), shoulder dystocia, difficult delivery, breech delivery, maternal history of previous delivery complicated by OBPP, prolonged second stage of labor, maternal diabetes, and considerable maternal weight gain during pregnancy (>22 kg). Recovery of biceps function within 3–6 months is of favorable prognostic significance. The more severe the lesion, the less likely the infant is to achieve a full recovery. Patients with delayed recovery may benefit from microsurgical interventions. Contractures, bone deformities, and joint instability in the shoulder and forearm may develop as a result of muscle imbalance in growing children. Patients with persistent weakness, contractures, and deformities may require reconstructive procedures.

  The 28th Annual Meeting of the European Bone and Joint Infection Society (EBJIS) was held in Vienna, Austria, from the 17th to the 19th of September, 2009. The convention was split into seven main panels, which included, in addition to presentations, 76 speeches and 33 posters, underlining the increasing significance of bone and joint infection pathology. This report is a summary of the aspects of the meeting that the author found to be of the greatest interest.

  The British Orthopaedic Association (BOA) Annual Congress returned to Manchester, UK, during September 15–18, 2009. Held at Manchester Central, formerly known as the Manchester International Convention Centre–GMEX, this 4-day event included a range of social and educational events. Specifically themed for Continuing Professional Development, the congress officially started with sessions on medico-legal issues, Article 14 access, and courses on “training the trainers”. The scientific program included instructional lectures delivered by a number of distinguished speakers from across the country, as well as free papers and posters. One of the highlights of this year’s congress was the “Best of the Best” sessions, which covered the best free papers to be presented.
