Editor’s note: In patients presenting with chest pain, the initial probability of coronary artery disease (CAD) coupled with an exercise tolerance test can determine which patients should be referred for invasive cardiac catheterization or non-invasive investigation (e.g. stress-echocardiography, myocardial perfusion scintigraphy, or cardiac magnetic resonance). The use of 64-slice coronary computed tomographic angiography (CTA) has recently been shown to be highly accurate in excluding coronary artery stenoses, with a negative predictive value in the region of 100% (Am J Cardiol 2007;99:925–9).
