Editor’s note: The current authors analyzed the occurrence and clinical outcomes of contact sensitization to topical anesthetics by patch testing 620 patients. Positive patch-test reactions to one or more of the topical anesthetics were noted in 25 (4%) of the patients. Of these 25 patients with positive reactions, 19 presented with or had a history of perianal dermatitis. Dibucaine (cinchocaine) reactions were the most common (20 out of 25 patients). A positive reaction to benzocaine without reaction to other anesthetics was shown in two patients, to prilocaine only in one, to procaine in another, and to lidocaine in a further patient. Concurrent reactions to dibucaine and other topical anesthetics were seen in six out of 20 patients.
