Editor’s note: The fetal insulin hypothesis (Lancet 1999;353:1789–92) proposes that the association between low birth weight (LBW) and later development of type 2 diabetes is a result of genetic variations that predispose to both characteristics, possibly as a result of reduced insulin secretion rather than the original suggestion of fetal malnutrition.
