Editor’s note: Aliskiren is the first of a new group of antihypertensive agents, the direct renin inhibitors, which act on the renin–angiotensin system at its rate-limiting step. Both angiotensin-converting enzyme inhibitors and angiotensin receptor blockers have been shown to be useful in the management of patients with diabetes and renal disease, particularly in reducing the level of proteinuria (

N Engl J Med 2008;358:2433–46). In this study, the authors compared the effects of aliskiren as monotherapy and in combination with irbesartan in type 2 diabetes patients with hypertension and significant proteinuria.
