Editor’s note: Chronic refractory pouchitis (CReP) develops in a minority of patients who undergo ileal pouch anal anastomosis (IPAA). This can be difficult to treat as the patient may relapse when antibiotic treatment is stopped, or may not respond to therapy. Some patients with CReP have inflammatory lesions in the jejunum and ileum that can be visualized with wireless capsule endoscopy (WCE), suggesting pan-enteric inflammation. The current authors report on the use of infliximab to treat the symptoms of pouchitis in this group of patients.
