Editor’s note: Collagenous colitis was only recognized approximately 25 years ago but seems to be increasing in prevalence (Gut 2007;56:504–8). Several medicines are often tried in this condition, including 5-aminosalicylic acid preparations, bismuth, and budesonide, as well as simple anti-diarrheals for symptom relief. Several clinical trials have shown the superiority of budesonide compared with placebo for the acute treatment of collagenous colitis, but relapses are relatively common and patients frequently have to go back to receiving medication.
