Editor’s note: Although tyrosine kinase inhibitors (TKIs) such as imatinib are the established first-line treatment for chronic-phase chronic myeloid leukemia (CML), secondary resistance develops at a rate of 4% per year (N Eng J Med 2006;355:2408–17). In advanced-phase disease, resistance develops in up to 90% of patients at 4 years. Second-generation TKIs, such as dasatanib and nilotinib, have been developed but secondary resistance is a problem.
