Editor’s note: The addition of rituximab to CHOP (doxorubicin, cyclophosphamide, vincristine, and prednisone) chemotherapy has improved the outcomes and survival of both younger (<60 years) and older (60–80 years) patients with diffuse large B cell lymphoma (DLBCL). Approximately 40% of DLBCL cases occur in patients aged >70 years and patient age remains an independent prognostic factor for DLBCL, functioning as a continuous variable for prediction of outcome. Inevitably, treatment-related toxicities increase with advancing age and the optimal treatment for the “fit elderly” patient remains uncertain.
