Editor’s note: Previous studies have estimated the incidence of central nervous system (CNS) relapse in aggressive lymphoma (excluding Burkitt and lymphoblastic histology types) to be in the order of 2–5% depending on the population studied. Certain risk factors have been shown to confer an increased incidence of CNS relapse, including elevated serum lactate dehydrogenase levels and the presence of more than one extranodal site of disease (Blood 1998;91;1178–84). However, there is no universal consensus as to which patients should receive CNS prophylaxis, or what the optimal preventative approach should be.
