Editor’s note: Multiple sclerosis (MS) has been traditionally considered an inflam-matory disease of the central nervous system. However, this view has recently been challenged in the medical literature with increasing recognition of the importance of the neurodegenerative component of thedisease, particularly in the more pro-gressive subtypes. The importance of the neurodegenerative aspect has been further supported by the apparent lack of efficacy of the potent immunomodulatory drugs used in relapsing–remitting and more pro-gressive subtypes of MS. The interaction between inflammation and neurodegeneration remains unclear.
