Editor’s note: In a previous study performed by the same authors, cognitive impairment at baseline was found to be associated with magnetic resonance (MR) parameters evaluating diffuse brain damage (J Neurol Neurosurg Psychiatry 2005;76:519–26). The aim of the present study was to establish whether diffuse brain damage outside of lesions, and their changes over 2 years, predicted the deterioration of cognitive performance over a 7-year period.
