Editor’s note: The results of several clinical trials have shown that approximately 11–15% of patients with a clinical diagnosis of Parkinson’s disease (PD) who were enrolled in these studies had normal 18F-Dopa positron emission tomography or β-CIT single photon emission computed tomography (SPECT) scans, suggesting a clinical misdiagnosis (Neurology 2005;64[Suppl 1]:A274. Ann Neurol 2003;54:692–3. Ann Neurol 2003;54:93–101. Arch Neurol 2004;61:1044–53). This conclusion was largely confirmed by subsequent clinical follow-ups and β-CIT SPECT data; however, the correct underlying diagnosis in these patients was not determined.
