Editor’s note: Diagnosis of the benign condition of Holmes–Adie syndrome is usually appropriate in patients with unilateral tonic pupil and tendon areflexia. In patients presenting with bilateral tonic pupils and tendon areflexia, the possibility of a generalized peripheral or autonomic neuropathy, such as widespread diabetic autonomic neuropathy with its poor prognosis, needs to be considered. This article presents a review of the pupillo-graphic results of 140 consecutive patients with bilateral tonic pupils. The authors document that the presence of anisocoria of >1 mm in the light, which probably reflects consecutive rather than simultaneous onset of pupillary dysfunction, and sector palsy can reliably identify Holmes–Adie syndrome in these subjects.
