Editor’s note: In some countries, carotid artery stenting (CAS) has rapidly gained popularity as an alternative to carotid endarterectomy (CEA) for the treatment of symptomatic and asymptomatic carotid stenosis. Randomized trials have produced conflicting results, with a number of industry-sponsored studies demonstrating non-inferiority of CAS, while other trials have demonstrated the converse, in one case to the extent of being stopped prematurely because of significantly worse outcomes in the CAS group (EVA-3S study; N Engl J Med 2006;19:1660–71).
