Editor’s note: Corneal graft surgery is often carried out in eyes with a complicated previous history. One dilemma for surgeons faced with corneal decompensation and an anterior chamber intraocular lens (ACIOL) is whether the presence of the ACIOL will compromise any transplant. In this study, the authors followed 25 eyes with an ACIOL that underwent Descemet’s stripping automated endothelial keratoplasty (DSAEK). All eyes had an endothelium to ACIOL distance of ≥3 mm. The donor lenticules were pulled through a 5.5-mm wound into the eye using a forceps. The mean preoperative endothelial cell density was 2606 cells/mm2.
