Endothelial keratoplasty: the relationship between donor tissue size and donor endothelial survival.

When performing Descemet’s stripping with automated endothelial keratoplasty (DSAEK), it has been assumed that inserting a larger corneal graft is better than inserting a small one because more endothelial cells will be transplanted. Terry et al. performed endothelial cell counts in eyes that had received either 8.0-mm or 8.5-mm DSAEK grafts. Counts were carried out at the central cornea and readings were taken over the first 2 postoperative years. All DSAEK grafts were inserted using folders. The size of the graft was not randomized but was based on the size of the host cornea and the surgical incisions placed in it.
