Treatment Strategies

Gainsbury ML, Chu DI, Howard LA et al. J Gastrointest Surg 2011;15:397–403.

These authors attempted to address the controversy surrounding the risk of surgical complications associated with preoperative infliximab use in medically refractory ulcerative colitis patients. No significant differences were identified in terms on infectious or non-infectious complications between an infliximab and non-infliximab-treated group, post-surgery. However, this study was limited by its retrospective design and the fact that all patients were operated on by a single surgeon.

There is considerable controversy as to whether preoperative use of infliximab increases the risk of surgical complications after proctocolectomy with ileal pouch–anal anastomosis (IPAA), which is currently the surgical procedure of choice for ulcerative colitis (UC) patients who are refractory to medical therapy.
