With >13 000 participants and 3539 submitted abstracts, the 2009 Annual Congress of the European League Against Rheumatism (EULAR 2009) was again a hugely successful and enjoyable meeting. The host city of Copenhagen in Denmark, even when drenched in rain, was welcoming, relaxed, and beautiful.

  Osteoarthritis (OA) is a major cause of pain and participation restriction. Recent evidence-based guidelines agree in terms of their core advice. Management of OA patients should be individualized and patient-centered, and nonpharmacological measures (including information access, exercise, weight reduction if overweight or obese, and reduction in adverse mechanical factors) are central to the management plan. A holistic approach to the patient, and optimization of contextual responses, will improve the effectiveness of any intervention for OA.

  “Flare” is a term that is commonly used by both patients and clinicians to describe a debilitating worsening of symptoms. As treatment strategies and medications become available to achieve low levels of disease activity and remission, measures to guide tapering of medications, or re-treatment when disease appears to worsen, are needed to facilitate clinical trials and to move these treatment approaches from clinical trials to clinical practice. An Outcome Measures in Rheumatology Clinical Trials (OMERACT) initiative, which includes researchers, clinicians, and patients, is now underway to evaluate these aspects of disease worsening in RA. Timely, data-driven completion of this project with an agreed definition of RA flare is intended to facilitate clinical trials, observational clinical research, and ultimately enhance clinical care.
