THERAPEUTICS RESEARCH

Appel GB, Contreras G, Dooley MA et al. J Am Soc Nephrol 2009;20:1103–12.

Intravenous cyclophosphamide (IVC), the standard treatment for lupus nephritis (LN), has been associated with ovarian failure and secondary malignancies. Consequently, there has been a need for the development of alternative therapies with less toxicity but at least equal efficacy to IVC. The results of previous clinical trials suggest that mycophenolate mofetil (MMF) is equal or superior to IVC in terms of safety and induction of LN remission. Data from the present large, international trial demonstrate the overall safety and efficacy of MMF to be comparable to IVC as an induction therapy in LN.

Mycophenolate mofetil (MMF) is an immunosuppressive agent that inhibits lymphocyte proliferation via a blockade of the enzyme inosine 5’-monophosphate dehydrogenase (an enzyme required in order for lymphocytes to generate the necessary purines for DNA/RNA synthesis) resulting in anti-inflammatory effects.
