Whipple’s Disease Presenting as Inflammatory Rheumatic Disease

A 61-year-old woman presented with bouts of painful blue and red discoloration of the hands. With the exception of a cough that the patient attributed to her heavy smoking, she reported no other complaints, in particular no ulceration, skin rash, dry eyes or mouth, sun allergy, hair loss, bowel disturbances, or diarrhea.

  Work loss due to rheumatoid arthritis (RA) and difficulties at work are important problems for people with arthritis and their families, healthcare providers, employers, insurance companies, and society at large. In this review paper, I will describe the impact of RA on employment, focusing on work loss due to arthritis, as well as reduced productivity while at work and other less tangible impacts of arthritis on employment. To prevent work loss from arthritis and address employment issues for people with arthritis, a good understanding of the problem of work loss due to arthritis is essential. For that purpose, I will review the literature on risk factors for work disability in RA and discuss the problems that people with arthritis encounter at work due to their arthritis.

  Gout is increasing in prevalence and is associated with frequent co-morbidities; however, it is still often poorly managed. This update reviews the importance of an accurate diagnosis of the condition, as well as the current status of treatment of acute flares and the long-term management of hyperuricemia. Outcome targets are emphasized along with new information on pathogenesis and emerging therapies.

  As always, the American College of Rheumatology (ACR) Annual Scientific Meeting delivered a wealth of new information and data on the treatment of rheumatoid arthritis (RA), inflammatory arthritis in general, and other autoimmune diseases. Many of the highlights involved new therapies, or additional information on existing therapies, and these will be reviewed below. Also of great interest was the unveiling of several new disease classification criteria, which will certainly prove important in the clinical decision-making that goes into selection of therapies in practice.
