Treating Rheumatoid Arthritis to Target: Background and Prospects

Josef S Smolen, MD, FRCP, and Daniel Aletaha, MD, MS

Among the chronic inflammatory rheumatic diseases, rheumatoid arthritis (RA) is not only the most frequent, affecting 0.5–1% of the population, but is also the one with the highest propensity for joint destruction. Even at the first visit of patients with early arthritis of <3 months’ symptom duration, approximately 10% can have radiographically visible signs of joint damage [1]; furthermore, within the first 2 years, evidence of joint destruction can be found in up to 70% of patients, sometimes affecting >50% of the maximally involved joints [2–4].
